Please fill out the form to register for Adult Soccer.

Rules

• Only individual entries are accepted, teams will be assigned with a mixture of all experience levels
• Small field size (half soccer field) WITH 4' by 6' goals AND No goalkeepers or protecting the goal (3 second rule)
• No offsides, All free kicks are direct and ABSOLUTELY No slide tackling
• No referees (however each team has a captain) and No unregistered players on the field
• Players are required to indicate their interest in participation for each applicable session by name, i.e. Co-Ed, Over-30 etc.
• Payment by check is preferred, but not required.
• Players are not admitted to the field on an ad-hoc schedule of payment. (No per week arrangements)
• Players are required to conduct themselves as well-behaved adults at all times. Physical or verbal misconduct may be grounds for dismissal.
• You are responsible for the supervision of your children on the premises.
• This is true at all times. No exceptions.
• Please bring your own water.

Waiver

I, the undersigned, in consideration of receiving permission from Fort Wayne Sport Club, 3102 Ardmore Avenue., Ft. Wayne, IN, to participate and play in a soccer league sponsored by the Fort Wayne Sport Club, do hereby release and forever discharge the Fort Wayne Sport Club, its officers, agents, employees, and others acting officially or otherwise, from any and all liability, claims, demands, actions or cause of action whatsoever, arising out of or related to any loss, damage or injury, including death, which may occur from any cause during my participation or play in the soccer league or any activities incident thereto.
